Payment and Insurance

Insurance:

• Blue Cross Blue Shield PPO Illinois (except for Marketplace Plans, reciprocal with all other US states)

• United Health Care PPO (Optum)

• USHIP (University of Chicago's student insurance)

• Aetna PPO (If your Aetna plan is with the Univeristy UCHP system, you must get a referral from your primary care provider in order to see our therapists. We can provide you with information needed to do so.)

• Cigna PPO

• Out of Network Plans: Lake Park Counseling can provide you with a SuperBill statement that you can submit to your insurance for possible partial out of network session reimbursements, depending on your plan.

** While we will always check your in surance benefits at the start of our work together, please always also check with your indiviudal plan prior to scheduling your first appointment with us, so you are aware of your plan's mental health coverage and any copay, coinsurance, or deductible fees.

Out of Pocket Payments and Reduced Rates:

• Our full out of pocket fees for Individual Psychotherapy and Couples Counseling are as follows:

  • Independently Licensed Therapist (LCSW, LCPC, PhD)

    • $150 for diagnostic intake session

    • $130 for subsequent individual sessions.

  • Provisionally Licensed Therapist, Under Clinical Supervision (LSW, LPC)

    • $140 for diagnostic intake session

    • $120 for individual psychotherapy sessions.

  • Masters Level Therpy Intern (non-licensed, with Masters in Counseling or Social Work under Clincial Supervision)

    • Between $35 to $115 per session based on Sliding Scale

Lake Park Counseling accepts cash, check, HSA and all major credit cards.
